All medical facilities are required to perform a background check on you to make sure you are eligible to practice and that there are no significant criminal charges that would prevent you from practicing.

Because of this a background disclosure form is required for the state you are applying to staff in and any states you have worked in the past or have lived in during the past 10 years.
